Kylie Harris is an experienced health, safety and risk leader who heads the HSEC function at an underground gold mine in New Zealand. With more than 25 years’ experience across mining, transport regulation, policing and defence, she brings a broad operational perspective to safety management systems, critical control management, operational assurance and incident investigation. Before moving to New Zealand, Kylie held senior leadership roles at one of Australia’s largest underground mining operations, where she led the health, safety, security, emergency response and risk functions. She has a particular interest in using data and digital tools to provide leaders with earlier, clearer insight into operational risk.

Christina Folley is a globally recognised mining safety leader with more than 25 years’ experience transforming health and safety outcomes across the resources sector. As Manager: Group Safety at First Quantum Minerals, she leads strategic initiatives in critical risk management, critical control assurance, safety culture and leadership capability across a diverse international mining portfolio. Christina previously served as Western Australia’s Director of Mines Safety and Chief Inspector of Mines, where she led major regulatory reform and strengthened safety performance across Western Australia. Combining regulatory authority with operational insight, Christina is passionate about practical, people-centred approaches that enable safer, more sustainable production.

Sean Lavin brings over 15 years of mining experience, combining frontline operational grit with strategic capability. His background as a heavy machinery operator and Site Safety and Health Representative in surface coal mining gives him a grounded, human-centred perspective that connects across all levels of an organisation. After completing a Master of Management (HR), Sean moved from the coal face into global corporate roles, developing a passion for Learning & Development and frontline leadership. Through Everyday Massive, Sean partners with mining leaders and teams to enhance workforce performance and translate compliance into capability. He is dedicated to helping organisations deliver change where it matters most.

Hendrik Lourens is a researcher and practitioner working at the intersection of production-system design, operational stability and mine safety. His central thesis is that the decade-long plateau in Australian mining fatalities is a predictable consequence of balanced-capacity production systems—and that flow-based operational redesign can restore the conditions on which sustained safety improvement depends. He is the author of Breaching the Mining Fatalities Plateau (Journal of Industrial Safety, 2026). At this conference, he presents the companion paper The Compliance Paradox, which applies this thesis to Queensland’s Resources Safety and Health Legislation Amendment Act 2024. He is also co-author, with Kelvin Genn of Art of Work, of Beyond Green Ticks, which introduces the Control Effectiveness Assessment methodology for evaluating whether critical controls work in practice. Hendrik is a Fellow of AusIMM and serves on the AusIMM Health and Safety Society Committee. He holds an MBA with Distinction from Heriot-Watt University, an MSc in Polymer Science from Stellenbosch University, a Certificate IV in Work Health and Safety, and international Theory of Constraints certification from TOCICO. As CEO of Stratflow Australia and a senior consultant at Art of Work, Hendrik has spent 12 years applying flow-based methods to safety and operational transformation in underground and open-cut mines across coal, metals and industrial minerals. His work builds on the Flow Room methodology developed by the late Arrie van Niekerk and proven through more than 90 operational interventions.

Peter works in high hazard industries including mining, oil and gas and chemical processing. In the UK he helped introduce safety cases following the Piper Alpha disaster (1988). Major incident investigations include BP Macondo/Deepwater Horizon disaster with the US Chemical Safety Board (2010) and many fires, explosions and fatal accidents. With ICMM he helped develop critical control guidance (2015) and a revised edition (2026). With this experience he helps companies improve their operational performance including safety. Peter sits on the Australian Government’s, Nuclear Safety Committee and the editorial board of the American Chemical Society’s, Chemical Safety Journal.
