Peter Standish (FAusIMM) is an experienced and qualified engineer with over 30 years of expertise in managing business challenges at various levels. He brings a scientific mindset to every task, solving problems through data-driven methods whenever possible. His extensive background working across different industries has equipped him with the ability to evaluate situations objectively and anticipate potential issues through analytical thinking and sound judgment. Peter is passionate about applying technological solutions to complex problems. His dedication to making technical information accessible to diverse audiences led to his involvement in creating the Risk Mentor platform and its associated data visualization tools.

Peter works in high hazard industries including mining, oil and gas and chemical processing. In the UK he helped introduce safety cases following the Piper Alpha disaster (1988). Major incident investigations include BP Macondo/Deepwater Horizon disaster with the US Chemical Safety Board (2010) and many fires, explosions and fatal accidents. With ICMM he helped develop critical control guidance (2015) and a revised edition (2026). With this experience he helps companies improve their operational performance including safety. Peter sits on the Australian Government’s, Nuclear Safety Committee and the editorial board of the American Chemical Society’s, Chemical Safety Journal.

Sean is a forensic engineer who investigates engineering failures from a technical and organisational perspective. In 2020, he completed the Brady Review, an investigation into the causes of fatalities in the Queensland mining industry. The review was tabled in parliament and made 11 recommendations to the regulator and mining companies on how to improve safety. In 2024, he completed the technical and organisational investigation into the catastrophic failure of a turbine generator at Callide C Power Station in Queensland, Australia. He also speaks, writes and podcasts on the causes of technical and organisational failure.

Anthony is the Director of Glasshouse Consulting and a trusted advisor on the design and delivery of operational risk and safety management practices to many tier one companies across a variety of high hazard industries. Anthony is widely regarded as a Thought Leader in the operationalising of fatality critical controls and a key contributor to the mining industries approach to preventing fatalities. He recently led the significant review and updating of the Critical Control Management Good Practice Guide for the ICMM. Prior to starting Glasshouse Consulting Anthony worked for Rio Tinto where he held several roles overseeing group programs relating to risk management, systems and assurance, including the design and deployment of their HSE Strategic Risk, Critical Risk Management and Process Safety Management programs. Prior to Rio Tinto, Anthony worked for Qest Consulting and GHD, and part of a team who leveraged their experience in working with oil, gas and chemical companies to help major mining companies gain a better understanding of their major hazards and shift their focus towards the effectiveness of risk controls. Anthony is passionate about ensuring the safety systems and leadership practices support the safe and efficient execution of work at the front line.

Christina Folley is a globally recognised mining safety leader with more than 25 years’ experience transforming health and safety outcomes across the resources sector. As Manager: Group Safety at First Quantum Minerals, she leads strategic initiatives in critical risk management, critical control assurance, safety culture and leadership capability across a diverse international mining portfolio. Christina previously served as Western Australia’s Director of Mines Safety and Chief Inspector of Mines, where she led major regulatory reform and strengthened safety performance across Western Australia. Combining regulatory authority with operational insight, Christina is passionate about practical, people-centred approaches that enable safer, more sustainable production.

Darren’s WHSE experience spans >40 years across a wide section of industries including Mining, Manufacturing, Construction, Utility services, Defence and Retail. He is a qualified geologist in Gold and Nickel where he worked as Chief Geologist with both WMC and Newmont. As a WHS professional, Darren has particular expertise in understanding an organisations risk profile, their critical risks and the development and implementation of effective controls that he sees as essential in the goal of improving safety performance and eliminating fatalities. He is a qualified Principal OHS auditor having audited around Australia and Internationally. Darren developed the 'Six Principles of Safety' model and is passionate about safety performance and the organisational, cultural and behavioural changes required to achieve sustainable safety performance improvement.

Simon is a qualified geologist and mining engineer with 50 years’ experience in the mining industry. He has experience in underground, open pit and regulation of mines in Africa and Australia. He has worked in copper, nickel, iron ore and gold operations. As a regulator in Western Australia and South Australia he has investigated multiple serious and fatal accidents including two multiple fatalities. In 2012 he was the recipient of the Jim Toolach Health and Safety Award recognising his work in driving positive changes in the Western Australian regulator. Currently he is the Independent Chair of the West Australian Mining and Petroleum Advisory Committee (MAPAC) the statutory advisory body set up to advise the minister on safety related issues current and emerging in Western Australia.
